CCC is a 501 c 3 organization created in 1987. It provides a unique approach to addressing root causes of poverty through an innovative integration of both a Social Service Department and an Economic Development Department:
Social Service Department provides client assistance on Monday, Wednesday, and Friday from 9 AM to 2 PM at their office located at 145 NE 4th Avenue, Boynton Beach, FL 33435 by appointment only! Walk-ins are welcomed on the 3rd week of the month for USDA, MASS FOOD, Emergency Pantry Assistance. Homeless may get special attention every Tuesday with soup, clothing, pick-up mail and food bags, and special counseling
Emergency Food Pantry
An emergency food pantry, monthly mass food distribution, affordable monthly food box distribution program, access to USDA food, application assistance for food stamps. Congregations, business, individuals, and organizations contribute non-perishable food items to stock the pantry. Community service groups and individual residents in Boynton Beach also support the food pantry through financial contributions. Contributions and our Urban Farming Project help to provide fresh fruit and vegetables to clients. Assistance with on-line applications for prescription drug, Medicaid, and SSI benefits CCC may provide emergency financial assistance to an individual or a family once a year (as funds are available for rent and utilities). CCC may provide vouchers for anyone who cannot afford to have a vital prescription filled and a voucher for 1-or 2-night stay if short term shelter is required.
Senior Caregiving to assist seniors to "Age In Place"
The Senior Caregiving Program was created to support low-income seniors to "Age In Place" with dignity while managing and mitigating nutrition-related health conditions through proper diet. Access to in-home, supportive care for seniors is extremely limited. CCC provides in-home services, connects seniors to a network of partners and resources, and engages them in managing their health conditions, treatment and medications through diet. Having developed a successful model in the Heart of Boynton, we have expanding that model, called the "Shalom Project," to the 33484 zip code area identified as another "Hidden Hunger" senior community in Palm Beach County. The program provides home delivered meals, chronic meal plans, veggie mobile deliveries, senior monthly food boxes, and social activities that include congregation meals, nutrition education and community involvement. Each week the center’s Senior Veggie Mobile takes fresh and locally grown produce to our frail and elderly shut-ins in the “Heart of Boynton” area (zip code 33435), Delray Beach (zip code 33484), & Boca Raton (zip code 33433). It includes the bounty from the urban farm along with produce from gleaning programs and fresh produce purchased from local farmers. The program offers an opportunity for case managers to provide a friendly visit, check for signs of malnutrition and dehydration, provide additional assistance (affordable food boxes, meals, registration assistance for SNAP benefits) if needed, and offer healthy, nutritious, & chronic food options.
